Most of these websites contain downloadable forms.   • Pharmacies that specialize in Workers Compensation
    Under the 2007 amendments to NY the NY Workers' Compensation Law, carriers, third party administrators and self-insureds are permitted to contract with a pharmacy or pharmacy benefits manager, in which case they can direct where you obtain prescriptions for medications related to on the job injuries. Not all carriers and self insureds have opted to do so. If your carrier or self-insured permits free choice of pharmacy you can go to any pharmacy that is willing to fill your prescription under the strict guidelines of the NY State Pharmacy fee schedule. This fee schedule pays the same as Medicaid, so many pharmacies will declineto handle prescriptions under Workers' Compensation.

    They will fill your prescription via mail order, if you are not able to visit them. They will deal directly with the insurance company so you don not have to. They will fill NYC Law Department cases.

    an organization that raises dollars to benefits those suffering visual and/or auditory impairments. Good information, featuring many events.

  • The following companies specialize in handling the pharmaceutical needs of injured workers. They will deal directly with the insurance company so you don't have to. They will even provide your medication if your compensation case is contested, as long as you continue to have an attorney prosecute the claim. They can not help you if your carrier does not permit free choice of pharmacy. They may not be able to fill all prescriptions because the fee schedule is apparently inadequate on some pharmaceuticals. You would have to speak to them directly.
